These are enhanced penalties from an ordinary refusal.  The only distinction is in the monetary fine:  first offense $533 to $633; second offense $633 to $1,133; and third offense $1,133.  All other consequences remain the same, including the total insurance surcharge of $3,000 payable in three annual payments of $1,000.
